We are seeking a Full-Time Case Manager.
The position is responsible for guiding participants through the transition from shelter/homelessness to permanent housing and self-sufficiency.
The Case Manager will advocate for the mission of the organization and promote the vision and direction of the organization by provide housing and income focused case management services to program participants in our supportive housing program.
This includes conducting assessments, providing advocacy, providing life skill development services, landlord-tenant mediation, service coordination, community resource linkage, and providing compassionate participant centered services to a diverse population.
DUTIES:
Provide direct case management services to program participants including but not limited to:
conducting intakes and assessments; developing participant-centered individual service plans; identifying supportive services; providing advocacy and support; demonstrating a healthy landlord-tenant relationship; and continuously educating program participants on their rights and responsibilities as a tenant Assists with the housing search and placement process by accompanying program participants to view units, advocating on behalf of program participants with landlords, and assisting program participants with locating valuable community and housing resources (i.
e.
, utility assistance, furniture assistance, etc.
) Engage with program participants on a consistent basis, appropriate to their prioritized needs, to identify their goals and supportive services needed to maintaining permanent housing, economic stability, and mental wellness Incorporate the program participants' goals related to housing, economic stability and mental wellness into an individualized service plan that is reviewed monthly and updated annually Obtains signed release of information forms and communicates regularly with agency staff and outside community organizations (i.
e.
, mental/behavioral health programs, substance use programs, and education programs, etc.
) Complete several HUD, agency and CoC required trainings within the first ninety (90) days of employment (i.
e.
, landlord-tenant laws, fair housing, housing first, life skill development, visual lead-paint identification, financial management, child abuse and neglect, etc.
) Creates reports, gathers statistics, and submits reports per guidelines Maintains participant and program records, including progress notes, in accordance with applicable standards and regulations Identifies and provides emergency crisis services according to agency policy and procedures Transports program participants according to agency policy and procedures as needed Minimum Requirements:
Bachelor's degree in Social Work or social service related field, with preferred social service experience.
An equivalent combination of experience and skills set will be considered.
Must have valid Driver's license, acceptable driving record, and deemed insurable Ability to maintain accurate records, and file timely participant progress notes in an electronic data system Ability to maintain a high level of ethical conduct regarding confidentiality, dual-relationships, and professional stature while ensuring a safe, professional, and supportive environment Available to attend trainings, meetings and community events as required.
Demonstrated knowledge of community resources, governmental program regulations and policies, and other social service providers available to eligible clients.
Demonstrated ability to obtain vital information during a personal interview to make proper recommendations.
Demonstrated ability to establish and maintain effective working relationships with service recipients, internal staff, providers, and other social service agencies.
