To provide daily activities where resident will achieve physical, intellectual and social activities to reach and maintain the highest level of functioning and quality of life.

Computer (Point Click Care) Copier Phone/PA System Television & VCR/DVD player Internet (YouTube, Netflix) Radio/CD Player Resident Rights Maintain the confidentiality of all resident care information including protected health information.
Report known or suspected incidents of unauthorized disclosure of such information.
Ensure that you treat all residents fairly, and with kindness, dignity, and respect Knock before entering the resident's room Report all grievances and complaints made by the resident to the Nurse Supervisor/Licensed Practical Nurse Report all allegations of resident abuse and/or misappropriation of residents property immediately to the Nurse Supervisor Communicate respectfully and appropriately, respect culture differences among residents and staff ESSENTIAL RESPONSIBILITIES AND DUTIES:
Maintain a working knowledge of established sanitary and infection control policies and procedures.
Notify Activity Director or Nursing Supervisor of any changes in resident's behavior.
Set Up, and lead/perform a variety of small and large group programs with the residents as assigned by the Activity Director.
Assist in planning, implementation, and evaluation of programs/activity calendar.
Provide activities in accordance with the resident's preferences which are stated on the activity assessment and care plan.
Work 1:
1 with residents who are unable or unwilling to come to planned group activity programs as specified on resident's care plan.
Ensure all material and supplies are available for the daily activities.
Escort residents to/from activities, dining room for lunch/dinner after programs are over.
Document daily (group and 1:
1) activity participation of all residents Complete baseline care plans for new residents.
Take and upload resident photos for their medical record Attend all in-service training as required.
To assist in arranging special programs, special events, and fundraisers within the department and with other departments.
Participate in special dress days (ex.
Hawaiian day, wear a special color day, etc.
) Distribute newspapers and mail residents.
If needed or the resident requests, agrees you can open and/or read mail to the resident and document.
Write all scheduled activity programs on the daily activity boards.
Maintain cleanliness of activity area, all storage areas, (closets) and office.
Ensures required supplies and materials are available for the assigned programs in a timely manner, prior to the start of programs.
Prepares activities in advance/prior to the time the activity is to begin.
Assist in disturbing monthly activity calendars & birthday signs.
Assist the receptionist with the phone system as needed.
Keep daily information whiteboard current.
Decorate/Undecorated environment for upcoming holiday/season.
Assist volunteers when needed.
Be a team player and have an enthusiastic approach.
